Text

(1)A pharmacy shall report in writing to the division not later than 10 business days:
(1)(a) before the date of:
(1)(a)(i) a permanent closure of the pharmacy facility;
(1)(a)(ii) a change of business name or ownership of the pharmacy facility;
(1)(a)(iii) a change of location of the pharmacy facility;
(1)(a)(iv) a sale or transfer of any controlled substance as a result of the permanent closing or change of ownership of the pharmacy facility; or
(1)(a)(v) any matter or occurrence that the division requires by rule to be reported; or
(1)(b) after the day on which:
(1)(b)(i) a final administrative disciplinary order is issued against the pharmacy license holder by the regulatory or licensing agency of the state in which the pharmacy is located if the pharmacy is a class D pharmacy;
