For purposes of this section:

(1)(1)(a) "Advertisement" includes:
(1)(a)(i) billboards;
(1)(a)(ii) written documents such as:
(1)(a)(ii)(A) brochures;
(1)(a)(ii)(B) pamphlets;
(1)(a)(ii)(C) direct mail solicitations;
(1)(a)(ii)(D) radio, television, and telephone solicitation scripts; and
(1)(a)(ii)(E) telephone directories;
(1)(a)(iii) media, including television, radio, and Internet websites; and
(1)(a)(iv) any other means of promotion intended to directly or indirectly induce a person to enter into an agreement for services with a health care provider.
(1)(b) "Advertisement" does not include materials that provide information about health care provider networks established by health insurance carriers.
(2)"Health care provider" means a natural person who is:
