(1)In accordance with Title 63G, Chapter 3, Utah Administrative Rulemaking Act, the department shall make rules regarding:

(1)(a) which vaccines are required as a condition of attending school;

(1)(b) the manner and frequency of the vaccinations; and

(1)(c) the vaccination exemption form described in Section 53G-9-304.

(2)The department shall ensure that the rules described in Subsection (1):

(2)(a) conform to recognized standard medical practices; and

(2)(b) require schools to report to the department statistical information and names of students who are not in compliance with Section 53G-9-302.
