Text

(1)An innovation program application shall include:
(1)(a) the name and a summary of the credentials of each teacher and other staff member who will be teaching or working in the opportunity class;
(1)(b) the name of each student whose parent has submitted a consent form consenting to the student becoming a participating student;
(1)(c) a description of the alternative curriculum and alternative classroom schedule, as applicable, that the innovation program application seeks approval for;
(1)(d) a description of how, when, and where the opportunity class instruction will take place and whether the instruction will include in-person, virtual, or hybrid components;
(1)(e) any other innovative curriculum or classroom schedule adjustments intended to be incorporated into the opportunity clas
