Text

(1)A parent of a K-12 student may give the parent's consent for the student to participate in an opportunity class by submitting the parent's written and signed consent, as described in Subsection (2), to the public school teacher who proposes to submit an innovation program application.
(2)(2)(a) A public school teacher who intends to submit an innovation program application shall provide a consent form to a parent of a prospective participating student.
(2)(b) A form by which a parent consents to the enrollment of the parent's child in an opportunity class shall contain:
(2)(b)(i) the name and a summary of the credentials of each teacher and other staff member who will be teaching or working in the opportunity class;
(2)(b)(ii) an explanation that the opportunity class is experimental
