§ 31A-8a-203 — Information filed with the department.
Utah·Title 31A Insurance Code·Ch. 31A-8a Health Discount Program Consumer Protection Act·Part 31A-8a-2 Licensure
(1)Prior to operating a health discount program, a person shall submit the following to the commissioner:
(1)(a) a copy of contract forms used by the health discount program for:
(1)(a)(i) health care providers or health care provider networks participating in the health discount program, including the discounts for medical services provided to enrollees;
(1)(a)(ii) marketing;
(1)(a)(iii) administration of the health discount program;
(1)(a)(iv) enrollment;
(1)(a)(v) investment management for the health discount programs; and
(1)(a)(vi) subcontracts for any services;
(1)(b) the program's proposed marketing plan; and
(1)(c) dispute resolution procedures for program holders.
(2)The company shall file prior to use:
(2)(a) the form of contracts used by the health discount program operator;
