§ 31A-30-114 — Disclosure.
Utah·Title 31A Insurance Code·Ch. 31A-30 Individual, Small Employer, and Group Health Insurance Act·Part 31A-30-1 Individual and Small Employer Group
(1)A covered carrier shall make the information described in Subsection (2) available:
(1)(a) to:
(1)(a)(i) a small employer; or
(1)(a)(ii) an individual; and
(1)(b) (1)(b)(i) at the time of solicitation; or
(1)(b)(ii) upon the request of:
(1)(b)(ii)(A) a small employer; or
(1)(b)(ii)(B) an individual;
(1)(c) as part of the covered carrier's solicitation and sales materials.
(2)The following information is required to be disclosed or made available under Subsection (1):
(2)(a) the provisions of the coverage concerning the covered carrier's right to change premium rates; and
(2)(b) the factors that may effect changes in premium rates;
(2)(c) the provisions of the coverage relating to renewability of coverage; and
(2)(d) the provisions of the coverage relating to any preexisting conditi
