§ 31A-30-104 — Applicability and scope.

Utah·Title 31A Insurance Code·Ch. 31A-30 Individual, Small Employer, and Group Health Insurance Act·Part 31A-30-1 Individual and Small Employer Group
(1)This chapter applies to any:
(1)(a) health benefit plan that provides coverage to:
(1)(a)(i) individuals;
(1)(a)(ii) small employers, except as provided in Subsection (3); or
(1)(a)(iii) both Subsections (1)(a)(i) and (ii); or
(1)(b) individual conversion policy for purposes of Sections 31A-30-106.5 and 31A-30-107.5.
(2)This chapter applies to a health benefit plan that provides coverage to small employers or individuals regardless of:
(2)(a) whether the contract is issued to:
(2)(a)(i) an association, except as provided in Subsection (3);
(2)(a)(ii) a trust;
(2)(a)(iii) a discretionary group; or
(2)(a)(iv) other similar grouping; or
(2)(b) the situs of delivery of the policy or contract.
(3)This chapter does not apply to:
(3)(a) short-term limited duration health insurance;
(3)(b
