Text

(1)(1)(a) An application for a license as a third party administrator shall be:
(1)(a)(i) made to the commissioner on forms and in a manner the commissioner prescribes; and
(1)(a)(ii) accompanied by the applicable fee, which is not refundable if the application is denied.
(1)(b) The application for a license as a third party administrator shall:
(1)(b)(i) state the applicant's:
(1)(b)(i)(A) Social Security number; or
(1)(b)(i)(B) federal employer identification number;
(1)(b)(ii) provide information about:
(1)(b)(ii)(A) the applicant's identity;
(1)(b)(ii)(B) the applicant's personal history, experience, education, and business record;
(1)(b)(ii)(C) if the applicant is a natural person, whether the applicant is 18 years of age or older; and
(1)(b)(ii)(D) whether the applicant has commit
