§ 31A-22-806 — Provisions of policies and certificates.

Utah·Title 31A Insurance Code·Ch. 31A-22 Contracts in Specific Lines·Part 31A-22-8 Credit Life and Accident and Health Insurance
(1)All credit life insurance and credit accident and health insurance shall be evidenced by an individual policy, or, in the case of group insurance, by a certificate of insurance delivered to the debtor.
(2)Each of these types of policies or certificates shall, in addition to satisfying the requirements of Chapter 21, Insurance Contracts in General, set forth:
(2)(a) the name and home office address of the insurer;
(2)(b) the identity, by name or otherwise, of the persons insured;
(2)(c) the rate, premium, or amount of payment by the debtor, if any, given separately for credit life insurance and credit accident and health insurance;
(2)(d) a description of the amount, term, and coverage, including any exceptions, limitations, and restrictions;
(2)(e) that the benefits shall be paid to
