(1)(1)(a) Except as provided under Subsection (2), an individual life insurance policy shall be reinstated upon written application made within three years, or within two years in the case of an individual life insurance policy with a face amount under $5,000, from the date of premium default.
(1)(b) An applicant described in Subsection (1)(a) shall
(1)(b)(i) produce evidence of insurability satisfactory to the insurer;
(1)(b)(ii) pay all premiums in arrears; and
(1)(b)(iii) pay or reinstate any other indebtedness to the insurer upon the policy, all with interest:
(1)(b)(iii)(A) compounded annually, at a rate not exceeding the rate set by the policy for policy loans compounded annually; or
(1)(b)(iii)(B) if no rate is set in the policy, the commissioner shall adopt a rule that sets the
