§ 26B-3-1001 — Definitions.

As used in this part:

(1)"Annuity" shall have the same meaning as provided in Section 31A-1-301.
(2)"Care facility" means:
(2)(a) a nursing facility;
(2)(b) an intermediate care facility for an individual with an intellectual disability; or
(2)(c) any other medical institution.
(3)"Claim" means:
(3)(a) a request or demand for payment; or
(3)(b) a cause of action for money or damages arising under any law.
(4)"Employee welfare benefit plan" means a medical insurance plan developed by an employer under 29 U.S.C. Sec. 1001, et seq., the Employee Retirement Income Security Act of 1974 as amended.
(5)"Health insurance entity" means:
(5)(a) an insurer;
(5)(b) a person who administers, manages, provides, offers, sells, carries, or underwrites health insurance, as defined in Section 31A-1-
