Text

(1)As used in this section:
(1)(a) "Eligible requester" means:
(1)(a)(i) a resident;
(1)(a)(ii) a prospective resident;
(1)(a)(iii) a legal representative of a resident or prospective resident; or
(1)(a)(iv) the department.
(1)(b) "Facility" means an assisted living facility or nursing care facility.
(1)(c) "Facility's leadership" means a facility's:
(1)(c)(i) owner;
(1)(c)(ii) administrator;
(1)(c)(iii) director; or
(1)(c)(iv) employee that is in a position to determine which providers have access to the facility.
(1)(d) "Personal care agency" means a person that provides assistance with activities of daily living.
(1)(e) "Provider" means a home health agency, hospice provider, medical provider, or personal care agency.
(1)(f) "Resident" means an individual who resides in a facility. (2
