(1)(1)(a) A person may not operate a DIYer used oil collection center or used oil collection center without holding a registration number issued by the director.
(1)(b) The application for registration shall include the following information regarding the DIYer used oil collection center or used oil collection center:
(1)(b)(i) the name and address of the operator;
(1)(b)(ii) the location of the center;
(1)(b)(iii) whether the center will accept DIYer used oil;
(1)(b)(iv) the type of containment or storage to be used;
(1)(b)(v) the status of business, zoning, and other applicable licenses and permits required by federal, state, and local governmental entities;
(1)(b)(vi) emergency spill containment plan;
(1)(b)(vii) proof of liability insurance or other means of financial responsibility i
