Text
(1)An owner or a manager of a residential vocational or life skills program shall annually register the residential vocational or life skills program with the division.
(2)An application for registration shall be on a form approved by the division and shall require:
(2)(a) the name, address, telephone number, email address, website, and facsimile number, if any, of the nonprofit corporation operating the residential vocational or life skills program;
(2)(b) the name and address of the registered agent of the corporation operating the residential vocational or life skills program;
(2)(c) the name, address, telephone number, email address, website, and facsimile number, if any, of the residential vocational or life skills program;
