Sec. 201.352. APPLICATION FOR REGISTRATION.

(a)A person required to register shall:
(1)file with the board a written application for registration; and
(2)pay, with the application, a registration fee to the board.
(b)The application must include:
(1)the person's full name, age, post office address, and place of residence;
(2)each place where the person is engaged in the practice of chiropractic;
(3)the college of chiropractic from which the person graduated; and
(4)the number and date of the person's license.
(c)On receipt of the application and registration fee, the board shall determine whether the applicant is licensed to practice chiropractic in this state based on the records of the board or other sources the board considers reliable. (c-1) On receipt of a renewal application
