Sec. 155.151. DUPLICATE LICENSE.

(a)If a license issued under this subtitle is lost or destroyed, the license holder may apply to the board for a duplicate license. The application must be on a form prescribed by the board, accompanied by an affidavit of the loss or destruction that states that the applicant is the person to whom the license was issued and other information concerning the loss or destruction of the license as required by the board.
(b)On payment of a fee set by the board, the board shall issue a duplicate license to the person.
