(a)For each divorce, dissolution of marriage, or annulment granted by any court in this state, a record shall be filed by the clerk of the court with the office of vital records and shall be registered, if it has been completed and filed in accordance with this section.
(b)The clerk of the court shall complete and forward to the office of vital records, on or before the tenth day of each calendar month, the record of each divorce, dissolution of marriage, or annulment filed with the clerk during the preceding calendar month. The form for a certificate of divorce shall contain a place for the recording of the divorced persons' social security numbers and such numbers shall be recorded on the certificate and any forms necessary to prepare such certificate.
