(a) (1) Any person desiring to take the state board written and oral examination shall make application in writing to the board on blanks provided for that purpose at least forty-five (45) days before the examination. (2) The application for the state board written and oral examination shall be accompanied by a nonrefundable application fee as set by the board. (3) The application for the national board examination shall be accompanied by a nonrefundable application fee as set by the board. (4) Any person desiring to take the clinical competency test shall make application in writing to the board on blanks provided for that purpose at least forty-five (45) days before the examination. (5) The application for the clinical competency test shall be accompanied by a nonrefundable application fee as set by the board. (b) (1) Applicants who are accepted for the state board written and oral examination shall pay a fee to the board at least forty-five (45) days prior to taking the examination, as set by the board. (2) Applicants who are accepted for the national board examination shall pay a fee to the board at least forty-five (45) days prior to taking the examination as set by the board. (3) Applicants who are accepted for the clinical competency test shall pay a fee to the board at least forty-five (45) days prior to taking the examination as set by the board. (c) The board shall give notice of the time and place of all examinations in such manner as the board may consider expedient.
