Text
(a)A professional music therapist certification issued under this part is valid for twenty-four (24) months from the date of issuance.
(b)A certified professional music therapist requesting renewal of a certificate must provide the following to the advisory committee in advance of the expiration date of the certificate:
(1)A completed application on a form provided by the advisory committee and payment of the fee set by the advisory committee; and (2) A certified or sworn statement to the advisory committee that the certified professional music therapist:
(A)Has completed at least forty (40) hours of continuing education prior to the expiration date of the certificate;
(B)Has not had a professional credential revoked, suspended, or restricted by the certification board for music thera
