The notarial certificate of a document executed pursuant to § 18-1-11.2 must be substantially in the following form: State of South Dakota County of ________________ ss On this ____ day of ___________, in the year ______, before me, ______________ (notary's name), the undersigned office appeared ______________ (signer's name) with a remote location of ______________ (city/state), whom I have personal knowledge by identity proofing and whom I positively identified as the person whose name is subscribed to the within instrument, appeared before me not in my physical presence but by means of a tamper-evident electronic notarization system, and I observed his/her execution of the same for the purposes contained therein and confirm that I affix my official seal to the same instrument so execute
