§ 5-37-26. Medical forms.
Physicians or other persons are prohibited from requiring patients or their representatives
            to sign blank forms provided by or for any third-party payers or nonprofit medical-service
            corporations prior to, during, or subsequent to rendering services. Any physician
            or other person violating any of the provisions of this section shall be fined up
            to one hundred dollars ($100) for each violation.
