A declaration may be in the following form or any other written form that expresses the wishes of a declarant regarding the initiation, continuation or refusal of mental health treatment and may include other specific directions, including, but not limited to, designation of another individual to make mental health treatment decisions for the declarant if the declarant is incapable of making mental health decisions: Mental Health Declaration. I, , having the capacity to make mental health decisions, willfully and voluntarily make this declaration regarding my mental health care.
