Text

(1)When insurance is the method chosen to prove financial responsibility, the certificate of insurance shall be signed by an authorized company representative and shall contain the following information:
(a)The date on which the policy was issued.
(b)The name and address of the named insured.
(c)The policy number.
(d)The amount of coverage in terms of the liability limits stated in ORS 682.105.
(2)The policy of insurance for which the certificate is given shall not be canceled or terminated except upon the giving of 10 days’ prior written notice to the Oregon Health Authority. However, an insurance policy subsequently procured and certified to the authority shall, on the date the certificate is filed with the authority, terminate the insurance previously certified with respect to any
