(1)A licensee conducting a race meet, before selling a mutuel wagering ticket or receipt to any person about whom there is any reasonable doubt of the person having reached the age of 18 years, shall require such person to make a written statement of age and furnish evidence of the true age and identity of the person. The written statement of age shall be on a form furnished by the Oregon Racing Commission, substantially as follows: ______________________________________________________________________________ Date _____ I am 18 years of age or over. _______________ Signature Evidence in support of age and identity: Driver’s License No._____ (_____) State Military Record No._____ Liquor Permit No._____ Other _____ (Fill in license or card number of any one or more of above) ______________
