Text

(a)Application for a license shall be made to the Commissioner on forms prepared by him and shall be under oath.
(b)The application for a license shall: 1. State the name of the applicant and the street address of his principal office, as well as the mailing address if such mailing address is different from the street address. 2. Contain evidence of the good moral character of the applicant, if the applicant is an individual, or of the partners, if the applicant is a partnership; if the applicant is a joint-stock association, trust, unincorporated association or corporation, the secretary or any assistant secretary thereof shall certify the name and address of each of the officers, directors, trustees or other managing officials, together with a designation of the office or offices held
