A.The State Board of Licensure for Professional Engineers and Surveyors shall keep a record of its proceedings and of all applications for licensing, which record shall show: 1. The name, date of birth and last-known mailing and email address of each applicant; 2. The date of application; 3. The place of business of the applicant; 4. The education, experience and other qualifications of the applicant; 5. The type of examination required; 6. Whether or not the applicant was rejected; 7. Whether or not a license was granted; 8. The date of the action of the Board; 9. The board-approved area(s) of competence in a specific discipline(s) or branch(es) of engineering; 10. A declaration under penalty of perjury from each applicant that he or she will abide
