§ 36-6149 — Annual business report.

A.Each year on or before March 1, every prepaid dental plan organization shallfile with the Commissioner a report of the business activities of said organization for the preceding calendar year. This report shall be signed by at least two principal officers of the corporation and said signatures shall be notarized.
B.Said reports shall be on forms prescribed by the Commissioner and shall include: 1. A financial statement of the organization, including a copy of the balance sheet and receipts and disbursements of the organization for the preceding year certified by an independent certified public accountant. The Commissioner may accept a full report of the most recent examination of a foreign prepaid dental plan, certified to by the appropriate examining official of another state; and 2.
