The bench warrant must be substantially in the following form: County of ...................... State of Oklahoma. To any sheriff, constable, marshal or policeman in this state: A B having been, on the ........ day of .........
A.D., 19...., duly convicted in the ......... court of the county of ........ of the crime of (designating it generally), you are therefore commanded forthwith to arrest the above-named A B and bring him before that court for judgment, or if the court has adjourned for the term, you are to deliver him into the custody of the sheriff of the county of ........
