A.Each application for an order authorizing or approving the interception of a wire, oral or electronic communication shall be made in writing upon oath or affirmation to a judge of competent jurisdiction and shall state the authority of the Attorney General to make such application. Each application shall include the following information: 1. The identity of the law enforcement officer initiating the application and the district attorney authorizing the application to the Attorney General; 2.
