The question submitted to the registered voters of each municipality shall be substantially in the following form: Shall the municipalities of ______ and ______ (name of all municipalities to be consolidated) consolidate as the ______ (city or town) of ________ and be operated under the _____________ form of government? ( ) Yes. ( ) No. If a majority of the votes cast in each municipality are in favor of consolidation, the governing body in each municipality shall declare, by ordinance, that the consolidation has been approved and shall proceed to consolidate under the terms of consolidation. The consolidation and formation of the municipal corporation shall take effect on the date named in the terms and conditions.
