The director of developmental disabilities shall adopt rules under section5124.03of the Revised Code to implement sections5124.50to5124.53of the Revised Code. The rules shall specify all of the following:

(A)The method by which written notices to the department required by sections5124.50to5124.53of the Revised Code are to be provided;

(B)The forms and documents that are to be provided to the department under sections5124.511and5124.512of the Revised Code, which shall include, in the case of such forms and documents provided by entering operators, all the fully executed leases, management agreements, merger agreements and supporting documents, and fully executed sales contracts and any other supporting documents culminating in the change of operator;

(C)The method by which
