Text

The licensee or administrator of a child care center, type A family child care home, or licensed type B family child care home, an in-home aide providing child care services, the director or administrator of an approved child day camp, and a border state child care provider shall keep a record for each eligible child enrolled with the center, home, in-home aide, camp, or provider, to be made available to the county department of job and family services or the department of children and youth on request. The record shall include all of the following:

(A)The name and date of birth of the child;
(B)The name and address of the child's caretaker parent;
(C)The name and address of the caretaker parent's place of employment or program of education or training;
