Text
(A)To be eligible to receive a community health worker certificate, an applicant shall meet all of the following conditions:
(1)Be eighteen years of age or older;
(2)Possess a high school diploma or the equivalent of a high school diploma, as determined by the board;
(3)Except as provided in division (B) of this section, successfully complete a community health worker training program approved by the board under section4723.87of the Revised Code;
(4)Not be ineligible for certification in accordance with section4723.092of the Revised Code;
(5)Not have committed any act that is grounds for disciplinary action under section3123.47of the Revised Code or rules adopted under division (F) of section4723.88of the Revised Code or, if such an act has been committed, be determined by the
