If a petition is filed under section4301.331of the Revised Code for the submission of the question set forth in this section, an election shall be held in the precinct as ordered by the board of elections under that section. The expense of holding the election shall be charged to the municipal corporation or township of which the precinct is a part. At that election the following question shall be submitted to the electors of the precinct: "Shall the sale of beer and intoxicating liquor at____________________ (insert the address of the permit premises) ____________________, which was adjudged to be a nuisance to the public by (insert the name of the court, including the name of the political subdivision of the court, issuing such judgment) ____________ on (the date the judgment was issued
