(A)The board of elections shall provide to a petitioner circulating a petition for an election for the submission of one or more of the questions specified in section4301.353or4301.354of the Revised Code, at the time of taking out the petition, both of the following:
(1)The names of the streets and, if appropriate, the address numbers of residences and business establishments within the precinct that would be affected by the results of the election;
(2)A form prescribed by the secretary of state for notifying affected permit holders of the circulation of a petition for an election for the submission of one or more of the questions specified in section4301.353or4301.354of the Revised Code.
