Text

(A)The department of health shall submit a report to the governor and, in accordance with section101.68of the Revised Code, the general assembly, evaluating the patient centered medical home program not later than three years after rules adopted pursuant to section3701.944of the Revised Code first become effective. The department shall submit a second report not later than five years after those rules first become effective.
(B)The reports submitted under division (A) of this section shall include all of the following:
(1)The number of patients receiving primary care services from certified patient centered medical homes and the number and characteristics of those patients with complex or chronic conditions. To the extent available, information regarding the income, race,
