Text

When a power of attorney created pursuant to section3109.52of the Revised Code terminates pursuant to division (A)(1), (2), (3), or (4) of section3109.59of the Revised Code, the grandparent designated as the attorney in fact shall notify, in writing, all of the following:

(A)The school district in which the child attends school;
(B)The child's health care providers;
(C)The child's health insurance coverage provider;
(D)The court in which the power of attorney was filed under section3109.74of the Revised Code;
(E)The parent who is not the residential parent and legal custodian and who is required to be given notice under section3109.55of the Revised Code;
(F)Any other person or entity that has an ongoing relationship with the child or grandparent such that the person or
