To create a power of attorney under section3109.52of the Revised Code, a parent, guardian, or custodian shall use a form that is identical in form and content to the following: POWER OF ATTORNEY I, the undersigned, residing at ___________, in the county of __________, state of __________, hereby appoint the child's grandparent, __________, residing at __________, in the county of ___________, in the state of Ohio, with whom the child of whom I am the parent, guardian, or custodian is residing, my attorney in fact to exercise any and all of my rights and responsibilities regarding the care, physical custody, and control of the child, __________, born __________, having social security number (optional) __________, except my authority to consent to marriage or adoption of the child _______
