Text
(A)Not later than ninety days after the effective date of this section, the department of health shall prescribe the following forms:
(1)A denial of release form to be used by a birth parent under division (A) of section3107.46of the Revised Code. The form shall explain that the birth parent may rescind the denial of release at any time by signing, dating, and having filed with the department of health an authorization of release form pursuant to division (B) of section3107.46of the Revised Code.
(2)An authorization of release form to be used by a birth parent under division (B) of section3107.46of the Revised Code. The form shall state that the birth parent may rescind the authorization of release at any time by signing, dating, and having filed with the department of health a denial
