Text
(A)As used in this section, "biological parent" means a biological parent whose offspring, as a minor, was adopted and with respect to whom a medical and social history was not prepared prior or subsequent to the adoption.
(B)A biological parent may request the department of children and youth to provide the biological parent with a copy of the social and medical history forms prescribed by the department pursuant to section3107.09of the Revised Code. The department, upon receipt of such a request, shall provide the forms to the biological parent, if the biological parent indicates that the forms are being requested so that the adoption records of the biological parent's offspring will include a social and medical history of the biological parent.
