Text
(A)Imposition of a three-year mandatory prison term upon an offender under division (B)(6) of section2929.14of the Revised Code is precluded unless the offender is convicted of or pleads guilty to violating division (A)(1) or (2) of section2903.06of the Revised Code and unless the indictment, count in the indictment, or information charging the offense specifies that the offender previously has been convicted of or pleaded guilty to three or more violations of division (A) of section4511.19of the Revised Code or an equivalent offense, or three or more violations of any combination of those offenses. The specification shall be stated at the end of the body of the indictment, count, or information and shall be stated in substantially the following form:
