Text
A written declaration executed by a declarant under section2108.70of the Revised Code shall be signed and dated by the declarant in the presence of one of the following:
(A)A notary public who shall make the certification described in section147.53of the Revised Code.
(B)Two witnesses who are adults and who are not related by blood, marriage, or adoption to the declarant.
(C)If the written declaration is a DD Form 93, Record of Emergency Data, by whomever the form requires.
