§ 2587. Program approval.

1.Eligible institutions seeking cash\nassistance under the program shall submit an application to the\ncommissioner. Such application shall be in a format and quantity\ndetermined by the commissioner, and shall include:\n (a) a general description of the proposed program;\n (b) a copy of a valid license or other approval issued by the\ndepartment, the department of social services or other state agency to\nprovide or sponsor child or adult day care programs; and\n (c) such other information as required by the commissioner.\n 2. The commissioner shall not approve an eligible institution for\nreceipt of cash assistance under this title unless the applicant:\n (a) accepts final administrative and financial responsibility for\nmanagement of an effective food
