§ 15. Reports by employer. Each employer of labor in this state who\nshall have entered into the plan with any employee shall, on or before\nthe first day of January, nineteen hundred and eleven, and thereafter\nand at such times as may be required by the industrial commission, make\na report to such commission of all amounts, if any, paid by him under\nsuch plan to injured employees, stating the name of such employees, and\nshowing separately the amounts paid under agreement with the employees,\nand the amounts paid after proceedings at law, and the proceedings at\nlaw under the plan then pending. Such reports shall be verified by the\nemployer or a duly authorized agent in the same manner as affidavits.\n
