§ 76. Notice of transitional services for incarcerated individuals\nreleased from correctional facilities. 1.

(a)Prior to the release of an\nincarcerated individual from a correctional facility, the department\nshall provide such incarcerated individual with information on\ntransitional services available in the county or city where such\nincarcerated individual is scheduled to be released. Such information\nshall include programs designed to promote the successful and productive\nreentry and reintegration of an incarcerated individual into society\nincluding medical and mental health services, HIV/AIDS services,\neducational, vocational and employment services, alcohol or substance\nabuse treatment and housing services.
