Text

1. The Board shall prescribe and furnish an application form for the use of all persons who desire to be licensed pursuant to this chapter. 2. All applications filed with the Board must be accompanied by all information required to complete the application and the required fee fixed by the Board in an amount not to exceed:

(a)For an administrator of a residential facility for groups, $150.
(b)For a nursing facility administrator, $250.
(c)For a health services executive, the amount prescribed by regulation of the Board. 3. The Board may fix and charge an additional fee to cover the cost of administering the examinations if the Board determines that there is not sufficient money from other sources to cover such costs.
