1. A person may file with the Commissioner a complaint alleging that another person has violated a provision of this chapter, a regulation adopted pursuant to this chapter or an order of the Commissioner. 2. A complaint filed pursuant to this section must:

(a)Be in writing;
(b)Be signed by the person filing the complaint or the designee of the person filing the complaint;
(c)Contain an address and a telephone number for the person filing the complaint or the designee of the person filing the complaint;
(d)Describe the nature of the alleged violation in as much detail as possible;
(e)Include as exhibits copies of all documentation supporting the complaint; and
(f)Include any other information or supporting materials required by the regulations adopted by the Commissioner or by an orde
