Text
When a licensed funeral director or funeral arranger has for any reason allowed the license to lapse, the Board may reinstate such license if application therefor is made within a period of 3 years from the time of the lapse and is accompanied by:
1.All fees, including penalties, from the time of the lapse to date of reinstatement; and
2.All information required to complete the reinstatement.
