Text

1. Each person desiring a license must apply to the Board upon a form, and in a manner, prescribed by the Board. The application must be accompanied by the fee for the application for an initial license and the fee for the initial issuance of the license prescribed by the Board, and all information required to complete the application. 2. The Board shall prescribe forms for applying for the issuance or renewal of a license. The forms must:

(a)Be available to be completed on the Internet website maintained by the Board;
(b)Provide immediate, automatic feedback to the applicant concerning whether the applicant has submitted all required information; and
(c)Automatically store the data submitted by the applicant upon completion of the application.
